- Research and Related Budget - generally used for subrecipient requests where a UTHSC PI proposes a subaward to an external institution and is included in a Cayuse proposal. External institutions will often provide their authorized budget in this format when they propose a subaward to UTHSC.
- PHS 398 Form Page 4 Detailed Budget for Initial Budget Period – may be used for internal account set up, budget reallocation requests, etc.

The following PHS 2590 forms may be requested in rare instances by NIH or another external sponsor as part of Research Performance Progress Reporting (RPPR). NIH generally requires RPPRs to be entered into eRA Commons; if a UTHSC PI is a subrecipient to an external institution’s grant, these forms may require institutional authorization.
