Replacement Diplomas Request
How to obtain a replacement diploma.
Replacement Diploma Policy:
- A replacement diploma will be issued if a notarized statement is sent to the Office of the University Registrar attesting that the original diploma was lost, destroyed, never received, or if the student had a legal name change.
- The name printed on the replacement diploma will be the same as the name printed on the original diploma unless there is a legal name change. If there is a legal name change, the original diploma and copies of any legal documents must be returned with this notarized statement (i.e. marriage license, divorce decree, driver’s license, social security card, etc.).
- The cost of the diploma is $20.00 plus $7.37 shipping. If it is to be shipped within TN, an additional $1.85 is required for tax. Check or money order should be made payable to University of Tennessee Health Science Center. Please allow 3-4 weeks for delivery of the replacement diploma. Send your request to UTHSC Office of the Registrar, 910 Madison, Ste. 520, Memphis, TN 38163.

Transcript Request
Transcripts can be ordered by current students, alumni, and former students of the University.
Current Students:

- By online through Banner Self Service (fastest). For instructions, see the Student Self Service Guide

Processing Time
Transcript requests are generally processed and mailed within 3-5 business days (5-7 during peak times). Transcript requests for alumni and former students who were at the University for any length of time before Fall 1991 will require additional processing time (anywhere from 5-10 days) as their secure records will be retrieved from University Archives.
Delivery Options
- Mail – Paper transcripts are mailed using standard delivery through the U.S. Postal Service. You may opt for express shipping at your own cost. Students must provide a pre-paid envelope with the recipient's name and address to the Registrar's Office.
- Pickup – Transcripts can be held for pick up at the Registrar's Office. Students must provide a photo ID before the transcript is released
